HOW TO REGISTER AT THE PRACTICE

You can register at the Practice if you live in our area.

You will need to fill in a questionnaire and bring in your medical card and a valid I.D. The Doctor will require you to come into surgery for a New Patient Check. You will be sent an appointment for this check through the post.

For the Practice catchment area, please ask Reception staff for a map.
